Why We No Longer Do Ultra-Rapid Detox

The Coleman Institute for Addiction Medicine has not performed an Ultra-Rapid Detox using anesthesia since 2001, when we developed The Coleman Method, a safer, faster, and more comfortable way to start your journey toward recovery.

What is Ultra Rapid Detoxification (URD) or Ultra Rapid Opioid Detoxification (UROD)?

Ultra-Rapid Detoxification is a medical technique that was pioneered in the 1980s to help people detoxify from opiates under anesthesia. Because older detoxification techniques were painful and usually unsuccessful, researchers were looking for a better approach. Today, this approach has many nicknames, including “rapid detox,” “anesthesia detox,” “sedation detox,” “rapid under sedation detox,” and “Anesthesia Assisted Rapid Opioid Detoxification (AAROD).”

URD involves putting patients under general anesthesia and giving them Naltrexone to flush all of the opiates off the brain’s receptors. It succeeds in doing so, but produces a major shock to the patient’s system. Patients usually feel absolutely terrible after the procedure and there are some risks involved. Approximately 1/500 to 1/1,000 patients die after the procedure and the exact cause of death is unknown. There are a number of centers around the world that still perform URD in a supervised setting with experienced personnel and it seems to be a reasonable procedure where the risks can be outweighed by the benefits. As of August 2022, we are not aware of any circumstances where Ultra Rapid Detox under general anesthesia is covered by health insurance in the US.

Why Do You No Longer Perform URD? Is Accelerated Detox Better?

The Coleman Institute performed over 300 URD procedures in a hospital and had no major complications. However, because of the risks involved, the patients’ discomfort level, and the high cost, we developed a new technique that would be just as successful and yet avoid the problems of URD. We have now performed thousands of Accelerated Opioid Detoxifications using The Coleman Method since 2001 and find the technique is vastly superior to our experience with URD.

Our Accelerated Opioid Detoxification procedure stabilizes patients for 2-3 days while gently flushing the opiates off the brain’s receptors. This allows the patient’s body to adjust much more comfortably and to lower risks. We avoid general anesthesia, which can be dangerous and has side effects of its own. Our procedure is safe enough to be performed on an outpatient basis, which helps to lower costs. Nevertheless, we have comparable success rates; approximately 98% of our patients have completed their detoxification and have started on Naltrexone therapy. Multiple patients who have had a rapid detox under general anesthesia and then have tried our newer Accelerated Opioid Detoxification technique have reported that our technique (the Coleman Method) is greatly superior.

Our Accelerated Opioid Detox protocol offers the benefits of URD but it is often safer, easier on the patient, and more affordable.
